With populations of around 250,000 to 500,000, Places cover a smaller geographical footprint and are typically served by health and care providers in a town or district, connecting Primary Care Networks to a broader range of services from local councils, hospitals and voluntary organisations. Some of the activities you might see here are a redesigning of local services, joining up care pathways across NHS, local government and the third sector and supporting the development of place-based partnerships and provider collaboratives.
